Measurement Strategy
The documents listed below provide information about the process and outcome measures for each clinical focus area that NYSPFP will submit to CMS in aggregate, in addition to details about the data collection methodology. Please contact your hospital’s NYSPFP Project Manager with any data-related questions.
Success Stories
Hospitals have submitted success stories to NYSPFP depicting sustained quality improvement that have occurred related to any hospital-acquired condition. Hospitals may submit run charts accompanied by brief narratives in any format.
Share Your Successes With NYSPFP
The Centers for Medicare & Medicaid Services (CMS) is soliciting hospital quality improvement examples demonstrating quantitative successes in hospital-acquired conditions and readmissions. Hospitals may submit hospital-specific data that are displayed as a run or control chart, depicting sustained quality improvement that have occurred related to any hospital-acquired condition. Hospitals may submit run charts accompanied by brief narratives in any format. For your convenience, NYSPFP is providing the template below as a guide. Feel free to use and/or modify the template as you see fit for sharing your successes. Please submit this information electronically to your NYSPFP Project Manager.
Patient and Family Engagement (PFE) Success Stories
The Partnership for Patients (PfP) is seeking best practices and success stories on effective patient and family engagement from patients, families, hospitals, and federal and private partners. The goal is to recognize those who are doing good work, as well as to identify practices that can be shared and emulated by other hospitals. The stories and best practices collected will be developed into case studies and shared among hospitals and other partners by the PfP PFE Affinity Group to support the rapid spread of best practices. Please submit this information electronically to your NYSPFP Project Manager.
No Harm Across the Board Success Stories
Hospitals may submit stories where they are intercepting harm and improving patient safety. This could be through overall good performance in multiple areas of success or it may focus on one innovation in one focus area. Stories should be data driven performance stories, identifying outcome measures showing zero outcome rates in any of the core focus areas, identifying high performance areas meeting benchmarks set for associated key metrics, or they can identify continued efforts in the reduction of "No Harm Across the Board".
