Preventable Readmissions Initiative

The Preventable Readmissions Initiative is designed to assist hospitals to plan and implement a focused initiative related to at least one area that has been identified as contributing to readmissions.

To reduce hospital readmissions by 12%. NYSPFP seeks to achieve this goal through aggregate improvement that will be tracked beginning with 2014 Potentially Preventable Readmission (PPR) data.

The primary goal of NYSPFP’s Readmissions Pilot Project is to reduce readmissions and examine key processes in the care delivery system for every patient admitted to or transferred into the pilot unit(s), throughout their hospital stay, and at discharge.
