Preventable Readmissions Initiative

The Preventable Readmissions Initiative is designed to assist hospitals to plan and implement a focused initiative related to at least one area that has been identified as contributing to readmissions.

Goal:
To reduce hospital readmissions by 20% by December 2014. NYSPFP seeks to achieve this goal through aggregate improvement that will be tracked beginning with December 2010 Potentially Preventable Readmission (PPR) data.
