Preventable Readmissions Initiative

The NYSPFP initiative to reduce Preventable Readmissions is designed to assist hospitals in identifying contributing factors to readmissions and implement focused interventions related to those areas. NYSPFP promotes reducing readmissions by transforming the patient care delivery system to actively include patients and care partners as members of the healthcare team.

Goal:
To reduce hospital readmissions by 12%.
Approach:
NYSPFP’s approach offers education and interventions to assist hospitals with implementing strategies to identify patients at risk for preventable readmissions and improve transitions of care.

NYSPFP’s Hospital-SNF Readmissions program identifies key factors in recurring readmissions between the participating facilities. NYSPFP provides resources to interested hospitals, including a comprehensive initial assessment, the NYSPFP Readmission Reduction through Partnerships with Skilled Nursing Facilities toolkit, and other improvement and evaluation components. Onsite and virtual coaching also is available through assigned Project Managers.

NYSPFP’s Care Partner Collaborative gives hospitals an opportunity to develop or enhance a care partner program to reduce readmissions and improve patient and care partner engagement. NYSPFP’s care partner education, coaching, and materials help hospitals to operationalize and hardwire care partner engagement throughout the hospital stay and beyond.
