Accelerating Improvement: Patient Safety Commitments:
NYSPFP has provided a framework to support hospitals as they continue to advance their performance improvement accomplishments. This framework includes specific patient safety commitments toward which hospitals will work and that focus on cross-cutting practices to improve safety that have the ability to impact a number of clinical conditions. Unit-Based Safety Posters:
The NYSPFP Unit-Based Safety laminated posters can be used at the unit or departmental level for frontline staff to track and document the number of days since the last harm event, as well as provide some strategies for reducing harm. The poster below is available for professional printing. It is 12"x18" (Full Bleed).
Data Tracking and Measurement
The following tool can be helpful for internal data tracking and to evaluate the impact of various interventions. The following document outlines the NYSPFP measurement strategy which, along with educational programming, is intended to provide hospitals with meaningful data to inform and help tailor interventions to reduce CLABSIs.
