Accelerating Improvement: Patient Safety Commitments
NYSPFP has provided a framework to support hospitals as they continue to advance their performance improvement accomplishments. This framework includes specific patient safety commitments toward which hospitals will work and that focus on cross-cutting practices to improve safety that have the ability to impact a number of clinical conditions.
NYSPFP Unit-Based Safety Posters
The NYSPFP Unit-Based Safety laminated posters can be used at the unit or departmental level for frontline staff to track and document the number of days since the last harm event, as well as provide some strategies for reducing harm. Please click on the following link for recommendations for how to effectively use the posters.
NYSPFP "Patient Safety Champions" Posters
Download, print and display the posters below throughout your hospital to encourage frontline staff and help spread the message for NYSPFP. There are six poster designs available: a general NYSPFP poster and five posters depicting the work of each NYSPFP initiative. The posters are legal paper size (8.5"x14") and can be printed on any standard office printer. The posters below are also available for professional printing. They are 11"x17" (Full Bleed). Download and send to a printer of your choosing to print larger versions.
Data Tracking and Measurement

The Centers for Medicare & Medicaid Services (CMS) is soliciting hospital quality improvement examples demonstrating quantitative successes in hospital-acquired conditions and readmissions. Hospitals may submit hospital-specific data that are displayed as a run or control chart, depicting sustained quality improvement that have occurred related to any hospital-acquired condition. Hospitals may submit run charts accompanied by brief narratives in any format. For your convenience, NYSPFP is providing the template below as a guide. Feel free to use and/or modify the template as you see fit for sharing your successes. If interested, please submit the following information electronically to your NYSPFP Project Manager.
