Injuries From Falls and Immobility

The NYSPFP initiative to reduce falls is designed to assist hospitals in implementing evidence-based strategies to prevent and reduce the incidence of all falls and associated injuries. NYSPFP focuses on identifying patients as high-risk for falls and injury and ensuring that all patients have access to fall prevention care.

Goal:
To reduce falls with injury by 20%.
Approach:
NYSPFP's approach includes applying best practices in fall prevention for frontline staff. Notably, NYSPFP supports hospitals to implement the Fall TIPS (Tailoring Interventions for Patient Safety) methodology by sharing tools and resources through webinars, coaching calls, and onsite support.

NYSPFP also provides tailored hospital-specific support with on-site and virtual coaching. Through the Rapid-Cycle Improvement Program (RCIP), NYSPFP assists multidisciplinary unit-level teams conduct individualized quality improvement projects in fall and injury prevention.
