Preventable Readmissions Educational Webconference: Doubling Down on Hospital Readmission Efforts

This Webconference will focus on identifying the underlying causes of readmissions and developing a hospital specific plan of action. NYSPFP staff will discuss the most recent 30-day Potentially Preventable Readmission (PPR) Diagnostic Reports and will highlight results from the Preventable Readmissions Assessment of Current Practices. Staff will also provide local examples of planning – and executing – targeted interventions to address preventable readmissions.

How to join the Webconference

Begin logging on 15 minutes prior to start time:

3. Enter the case-sensitive password: Change123
4. Click the "Join Now" button.
5. Fill in any additional information requested.
6. Click the "Submit" button at the bottom right-hand side of your screen.

After loading, a pop-up box will appear with the following dial-in instructions:

• The Meeting ID number: 641 453 538
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.
