Strategies for Reducing Anticoagulant-Associated Medication Errors

This webinar will focus on evidence-based safety practices for the management of high alert medications, and in particular, anticoagulants. Since anticoagulants are among the most common medications that cause adverse drug events, the Webinar will highlight quality improvement successes in reducing anticoagulant-associated medication errors in hospitalized patients. Steven Meisel, Pharm.D., CPPS, Director of Patient Safety at Fairview Health Services in St. Paul Minnesota will join this Webinar to present on successful strategies in monitoring and managing anticoagulants.

How to join the Webconference

2. Enter your name and e-mail address
3. Enter the case-sensitive password: Alert123
4. Click the “Join Now” button.
5. Fill in any additional information requested.
6. Click the “Submit” button at the bottom right-hand side of your screen.

After loading, a pop-up box will appear with the following dial-in instructions:

• The toll-free phone number: (877) 668-4493
• The Meeting ID number: 646 980 467
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.

This Webconference will be presented with Closed Captioning.
