Preventable Readmissions Educational Webconference: What is a Preventable Readmission? Stratifying Patients at Risk for Readmission

This Webconference will focus on identifying and stratifying patients at risk for readmission. Dr. Gregory Maynard, Clinical Professor of Medicine and Chief of the Division of Hospital Medicine at UC San Diego Medical Center, will highlight risk stratification strategies from Project BOOST, including the widely recognized 8P’s Risk Assessment Scale.

How to join the Webconference

Begin logging on 15 minutes prior to start time:

2. Enter your name and e-mail
3. Enter the case-sensitive password: Risk123
4. Click the "Join Now" button.
5. Fill in any additional information requested.
6. Click the "Submit" button at the bottom right-hand side of your screen.

After loading, a pop-up box will appear with the following dial-in instructions:

• The toll-free phone number: (877) 668 4493
• The Meeting ID number: 647 918 501
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.

This Webconference will be presented with Closed Captioning.
