Preventable Readmissions Educational Webconference

This educational Webconference is designed to provide hospitals with further context for the NYSPFP Preventable Readmissions Initiative as part of the Centers for Medicare & Medicaid Services’ (CMS) national Partnership for Patients (PfP) goals. Joshi S. Maulik, Dr.P.H., President of the Health Research & Educational Trust (HRET) and Senior Vice President of Research at the American Hospital Association (AHA) will discuss the evidence behind effective strategies to prevent avoidable readmissions. Representatives from CMS will provide context for the national PfP initiative and their goals to reduce all hospital readmissions by 20% compared to 2010.


The Webconference will feature Institute for Healthcare Improvement faculty.

Begin logging on 15 minutes prior to start time:

4. Click the "Join Now" button.
5. Fill in any additional information requested.
6. Click the "Submit" button at the bottom right-hand side of your screen.
After loading, a pop-up box will appear with the following dial-in instructions:
• The toll-free phone number: (877) 668 4493
• The Meeting ID number: 645 353 152
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.
