The Power of the Care Partner: A Special NYSPFP Event

NYSPFP invites hospital members to participate in an interactive, in-person/live-streamed meeting on becoming a “care partner hospital.” As we all know, engaging the care partner leads to better outcomes, a better patient experience, and aligns with legal and regulatory requirements. Yet, hospital leaders struggle to get traction and build momentum for redesigning care delivery to support the people who care for the patient after discharge. NYSPFP planned this special event to bridge the gap between conceptualizing and implementing a care partner program.

At this meeting, Amy Boutwell, MD, MPP, will present on how to utilize NYSPFP support and resources to include the care partner as a key member of the healthcare team. The event will also feature Michelle Van De Graaff, RN, BSN, from Intermountain Medical Center, who will share her experience implementing Partners in Healing®, a successful care partner program that positively impacted patient outcomes on the unit, hospital, and system level.

Participants at the live event will have an opportunity to interact and network with the speakers and other hospital participants that have or are considering adopting the care partner model.

Speakers

• Dr. Boutwell is the President of Collaborative Healthcare Strategies and a national expert on delivering patient-centered care and preventing readmissions
• Ms. Van De Graaff is Practice Chair of the Cardiovascular Unit at Intermountain Medical Center in Utah and is the creator of Partners in Healing®.

NYSPFP strongly encourages nursing leadership, unit leadership, frontline staff, CMO/physicians, patient and family engagement leaders, care transitions teams, unit-based safety champions, nurses, therapists, environmental services, and quality improvement leaders to participate in this event.
