NYSPFP Care Partner Collaborative Webinar: Commit and Identify

NYSPFP is pleased to invite members to participate in the first webinar in our new collaborative to engage and optimize care partners. Enrolling in NYSPFP’s Care Partner Collaborative gives hospitals an opportunity to focus on the development or enhancement of a care partner program.

The first of three webinars in the Collaborative, the July 24 event will focus on operationalizing the first two steps of the NYSPFP Care Partner Framework, Commit and Identify.


Amy Boutwell, MD, MPP, President of Collaborative Healthcare Strategies and NYSPFP faculty for this webinar series, will discuss the first two key steps, Commit and Identify.


Participants will learn the building blocks for implementing a successful care partner program at their hospital. 

Step 1: Commit—Become a Care Partner Hospital
Key strategies addressed:
• Dedicate a program leader
• Establish a care partner program 
• Broadly promote the care partner role 
• Continuously evaluate and improve the program

Step 2: Identify—Patients Choose Their Care Partner
Key strategies addressed:
• Support patient to identify a qualified care partner
• Introduce care partner to the medical team
• Display name and contact info in highly visible areas
• Provide a visual identifier for care partner to wear in the hospital
