NYSPFP Readmissions Webinar: Call to Action Recap and Future Care Partner Collaborative

NYSPFP invites you to join a March 20 statewide webinar, which is in follow up to the recently completed Call to Action: Effectively Engaging Patients and their Care Partners series of onsite coaching sessions at hospitals across the State. This webinar will also announce an opportunity to join a future NYSPFP Care Partner Collaborative. Each Call to Action event focused on preventing readmissions by redesigning care processes to engage both the patient and their designated care partner as members of the healthcare team.

Care Partner Collaborative
Care partner programs, such as those developed by Planetree, are evidence-based interventions that have a positive impact on reducing readmissions and increasing HCAHPS (Hospital Consumer Assessment of Healthcare Providers and Systems) scores. Aligned with the New York State CARE (Caregiver Advise, Record, Enable) Act regulations and best-in-class patient-centered care approaches, the implementation starts upon entry into the hospital and follows the patient’s smooth transition to home and post-hospital care services.

During the webinar, NYSPFP will invite hospitals to participate in its upcoming Care Partner Collaborative focused on this important work to redesign care delivery.

Speakers

Facilitating the events was Amy Boutwell, MD, MPP, and President of Collaborative Healthcare Strategies, who will once again join NYSPFP as faculty for this webinar. Dr. Boutwell will discuss action steps identified as meaningful (meaning having high impact on outcomes) by hospitals and feasible for engaging care partners during the coaching sessions and describe the next steps hospitals can take in adopting this approach.
