NYSPFP Readmissions Webinar: Reducing Readmissions through Collaboration with Skilled Nursing Facilities

On December 10, NYSPFP will host a webinar on reducing readmissions from skilled nursing facilities (SNF). This year, NYSPFP conducted a SNF Readmission Reduction collaborative pilot with a core group of hospitals. The pilot tested tools and strategies to establish and sustain a foundation for ongoing collaboration with SNF partners as part of an overall strategy to reduce readmissions. Data from the pilot shows a 20% improvement in the readmission rates of participating hospitals over the year. The pilot instituted a hospital and SNF collaborative to implement key interventions designed to strengthen relationships, standardize information transfer, and engage patients and care partners in care transitions.


The webinar will feature speakers from Wyckoff Heights Medical Center, IPRO, and NYSPFP. Representatives from Wyckoff, which participated in the pilot, will share successes and best practices from their collaborative with SNF partners. IPRO staff will discuss the importance of medication reconciliation across the care continuum and a tool to assist in capturing improvement opportunities in medication reconciliation within a collaborative. NYSPFP staff will review new tools and resources that were developed as a result of lessons learned during the pilot.

NYSPFP encourages the participation of multidisciplinary hospital staff, including discharge planners, care transitions coaches, care managers, pharmacists, patient engagement directors, emergency department staff, nurse managers, and other quality and safety leaders. NYSPFP also encourages hospitals to invite SNF partners.
