NYSPFP Healthcare Disparities Webinar: A Framework for Addressing Social Determinants of Health and Preventing Readmissions

On September 27, NYSPFP will present A Framework for Addressing Social Determinants of Health and Preventing Readmissions, the second webinar in our Healthcare Disparities initiative series focused on building and sustaining an organizational response to the problem. The National Quality Forum (NQF), in collaboration with the Centers for Medicare & Medicaid Services (CMS), describe social determinants of health (SDH) as “the most influential factors that affect the health of individuals.”

As such, healthcare organizations have a critical role in strategically addressing SDH barriers to prevent readmissions, focusing on system-wide redesign and targeted efforts aimed at preventing readmissions among minority and vulnerable populations. This NYSPFP webinar will provide a framework for healthcare organizations to respond to disparities by implementing and adopting a QI (quality improvement) effort to identify SDH, reduce readmissions, and improve healthcare quality for all using CMS’s Guide to Preventing Readmissions Among Racially and Ethnically Diverse Medicare Beneficiaries.

Speakers

The webinar will feature national expert Aswita Tan-McGrory, MBA, MSPH, Deputy Director of the Disparities Solution Center at Massachusetts General Hospital, who will discuss best-practice guidance, including:
• Understanding the role of SDH in addressing healthcare disparities
• Leveraging SDH data to improve patient outcomes
• Detailing an overview of key issues, risk factors, and strategies to prevent readmissions among diverse populations
• Creating a framework for identifying social determinants and populations at risk for disparities

Additionally, NYSPFP staff will share tools and resources available to support hospitals’ efforts to improve equity and address health disparities.

Who Should Attend

NYSPFP liaisons, hospital quality and safety staff, diversity and inclusion leaders, nursing and physician leaders, and any staff responsible for addressing health equity and healthcare disparities are encouraged to participate. Staff responsible for training, developing, or overseeing patient demographic data collection and reporting processes are also encouraged to attend.
