NYSPFP ADE Webinar in the Optimizing Anticoagulation Care Series: Transitions of Care for Patients on Anticoagulants

NYSPFP is pleased to announce the final webinar in its Optimizing Anticoagulation Care series, presented in collaboration with the Society of Hospital Medicine (SHM). The September 20 webinar, Transitions of Care for Patients on Anticoagulants, will provide hospitals with information and strategies to support their VTE (venous thromboembolism) prevention activities. Expert faculty will discuss the key components of preparing patients on anticoagulants for discharge, including patient and family education, community linkage, and the transfer of information to the next care provider.

NYSPFP encourages multidisciplinary hospital staff responsible for VTE prevention activities—including hospitalists, intensivists, pharmacists, nurses, nutritionists, and other quality and safety leaders—to participate in the webinar.
