NYSPFP Webinar: Readmission Reduction through Partnerships with Patients and Families: Strategies for Identifying the Care Partner

On March 13, NYSPFP will host a webinar focused on strategies and best practices for identifying and engaging patients and care partners. The webinar will help hospitals operationalize a process that meets state and federal requirements such as the New York State CARE (Caregiver Advise, Record, and Enable) Act, which requires hospitals to provide adult inpatients, who will be discharged to home, the opportunity to designate a "caregiver/care partner" who can assist the patient in their care after discharge.

This webinar will present strategies and best practices that facilitate identifying a patient’s caregiver/care partner, through obtaining consent for information sharing, informing patients and caregivers about discharge and post-acute care.

Who Should Attend

The sessions are for multidisciplinary hospital staff responsible for discharge planning before, during, and after an acute care encounter. NYSPFP encourages nursing, care management, and patient experience leadership, medical staff, nurses, patient and family advisory council members, and other quality and safety leaders to attend.
