NYSPFP Readmissions Domain II Webinar: Improving Processes to Reduce Readmissions

Join NYSPFP for this webinar on processes to ensure smooth transitions between acute care and skilled nursing facility (SNF) settings.

The featured speakers will present their implementation strategy in reducing readmissions from the SNFs to whom they discharge their patients. They will also discuss multicomponent process improvement interventions that produced positive results. The work emphasizes the partnership between the two facilities, and there is no better time than now for your hospital to partner with your SNF network.
