NYSPFP Regional Conferences: Sepsis Improvement Science

10/24/2017, 10/25/2017, 10/26/2017, 11/7/2017, 11/8/2017

NYSPFP is pleased to announce Sepsis Improvement Science, five regional, in-person conferences to be held at various locations throughout New York State in collaboration with Northwell Health’s Clinical Transformation division.

The conferences will incorporate hands-on learning in a small group environment, allowing for one-on-one coaching with our guest experts to improve sepsis outcomes. Attendees are encouraged to bring their facility’s sepsis protocols, policies, and any hospital-specific algorithms to help your team develop hospital-specific improvement processes.

Northwell Health is a member of the High Value Healthcare Collaborative, a national consortium of hospitals and health systems with whom NYSPFP has been working that completed a three-year sepsis improvement program to improve processes, outcomes, and costs associated with sepsis care. Northwell will provide faculty to present the latest quality improvement science and evidence-based best practices for identifying and treating sepsis. The conference will provide ample networking and break-out periods to share and consider various improvement techniques and tactics.

Poster Presentation
Each conference will also feature a poster presentation gallery walk. NYSPFP invites you to share your team’s experience implementing a hospital-wide quality improvement project on sepsis. The poster presentations provide an opportunity for hospitals to showcase their sepsis improvement work to hospital leadership and colleagues, and serve as an effective internal educational tool.

What to Share
Poster presentations should tell the story of a hospital’s successes, challenges, and lessons learned, along with the supporting data from implementing a sepsis quality improvement project.

Share your story with NYSPFP Support—just fill in the template and NYSPFP will do the rest!
• Use NYSPFP's PowerPoint Template to organize your hospital's information, data, and highlights.
• Access the NYSPFP Companion Guide for additional guidance and instructions on how to create your presentation.
• E-mail completed PowerPoint slides to learning@hanys.org by Friday, October 6, 2017.

Once received, NYSPFP will print out your hospital’s submitted slides and assemble them into a storyboard that will be displayed as a gallery walk at each of the five conference locations. You only need to attend the event that is most convenient to you. NYSPFP will deliver and display each hospital's storyboard at all five sites to ensure that all locations benefit from the presentations. Submission of the presentation materials gives NYSPFP permission to display the information at all of the Sepsis Improvement Science Conference locations, on the NYSPFP website, and share with other hospitals upon request.

Who Should Attend

NYSPFP encourages all members of hospitals' multidisciplinary sepsis and quality improvement teams to attend.
