NYSPFP Readmissions Reduction Webinar: Improving Transitions of Care between Acute Care Hospitals and Skilled Nursing Facilities

Is there a high readmission rate among patients you discharge to post-acute care providers, such as skilled nursing facilities (SNF)? Or, do a high percentage of patients sent to the emergency department (ED) from your SNF get admitted to the hospital? One of the best ways to reduce readmissions is to improve the transition from the hospital to the SNF and prevent an avoidable admission following appropriate treatment in the ED.

On September 28, NYSPFP will host a readmissions reduction webinar, Improving Transitions of Care between Acute Care Hospitals and Skilled Nursing Facilities.

Speakers

The webinar will feature Amy Boutwell, MD, MPP, and President of Collaborative Healthcare Strategies. Dr. Boutwell will present key processes for reducing readmissions by improving the transfer of patients from the ED to the nursing home, such as:
• Building and strengthening relationships across care settings
• Creating a communication and information exchange process for transfers from the hospital to the nursing home
• Engaging the patient, family members, and caregivers/“care partners” in the transfer process
