NYSPFP Regional Conferences: Readmission Reduction Through Partnerships with Patients and Families

NYSPFP is pleased to announce Readmission Reduction Through Partnerships with Patients and Families, five regional, in-person conferences to be held at various locations throughout New York State. These events will focus on helping your hospital reduce the likelihood of readmission by giving your staff the tools to help facilitate a successful discharge plan with patients and care partners.

NYSPFP is organizing these conferences across the state to recognize and promote the emerging practice of integrating care partners, along with the patient, into the discharge planning process. Patient and care partner engagement is shown to reduce health care costs and resource utilization, improve patient experience, and prevent readmissions. The conferences will focus on strengthening discharge systems, processes, and practices with the patient, family, and care partners at the center of the discharge planning process.

Speakers

Regional sessions will include presentations by top national experts Dr. Amy Boutwell, MD, MPP of Collaborative Healthcare Strategies, and Beverly Johnson, President of the Institute for Patient and Family-Centered Care, who will share evidence-based best practices and materials for a more integrated approach with patients and care partners before discharge. NYSPFP Project Managers will also provide support for your team’s action plan development in this area to assist with your readmissions reduction goals.

Who Should Attend

The sessions are for multidisciplinary hospital staff responsible for discharge planning before, during, and after an acute care encounter. NYSPFP encourages nursing, care management and patient experience leadership, medical staff, nurses, Patient and Family Advisory Council members, and other quality and safety leaders to attend.
