NYSPFP Preventable Readmissions Initiative: Webinar Focused on Effective Discharge Planning

On April 4, 2017, NYSPFP will host a webinar focused on engaging patients and care partners in the discharge planning process. The webinar will help hospitals meet State and Federal requirements such as the New York State CARE (Caregiver Advise, Record, and Enable) Act, which requires hospitals to provide adult inpatients, who will be discharged to home, the opportunity to designate a "caregiver" who can assist the patient in their care after discharge.

During the Webinar, Carol Levine, Director of the Families and Health Care Project of the United Hospital Fund, will present a recently published Toolkit on Implementing the New York State CARE Act. The Toolkit details the four key requirements of the CARE Act, from offering patients the opportunity to identify a caregiver through obtaining consent for information sharing, informing patients and caregivers about discharge options, and providing needed instruction on home care tasks.
