NYSPFP Preventable Readmissions Kick-off Webinar

NYSPFP will host a statewide webinar focused on preventable readmissions at 1:00 p.m. on Wednesday, January 25. This webinar will introduce the Hospital Improvement Innovation Networks (HIIN) Preventable Readmissions Initiative which is focused on:

1. Redesigning the discharge process to engage patients, families, and caregivers in alignment with New York State and Federal regulations such as New York State’s Caregiver Advise, Record and Enable (CARE) Act.
2. Improving care transitions between hospitals and skilled nursing facilities, utilizing a regional collaborative approach with 8 – 10 hospital and nursing home partnerships.

This webinar will feature Amy Boutwell, MD, MPP, founder of Collaborative Healthcare Strategies. At Collaborative Healthcare Strategies, Dr. Boutwell focuses on best practice approaches to improve care transitions, without exclusive adherence to one particular model—taking the best from what is known to be effective, practical, and efficient in improving care transitions. During the webinar, Dr. Boutwell will share her experiences and innovative approaches to complement the NYSPFP preventable readmissions work.
