Readmissions Webconference: Pilot Phase 3 and Preventing Readmissions at the Point of Discharge and Beyond

NYSPFP is pleased to feature Mark V. Williams, M.D., Director, Center for Health Services Research, Professor and Vice Chair Department of Internal Medicine, and Interim Chief, Division of Hospital Medicine at the University of Kentucky, who will discuss critical communication strategies at discharge to ensure safe transitions across the continuum of care to mitigate risk for readmission. Dr. Williams will also share relevant tools and resources for hospital staff, patients and family/caregivers to promote a comprehensive patient-centered discharge plan.

Also on the call, NYSPFP staff will provide an overview of Phase 3 of the Preventable Readmissions Pilot Project which focuses on key activities at discharge including, ensuring patient and family/caregiver are fully prepared for post hospital care and providing timely and thorough communication to post hospital providers. NYSPFP staff will discuss key strategies for ensuring an effective discharge and provide tips for implementation in your hospital to reduce the risk for readmissions.

NYSPFP encourages all hospitals, including those not participating in the readmission reduction pilot project, to attend.
