The Integration of Safe Practices Related to Anticoagulation Management

On May 7, NYSPFP will host a webinar focused on safe anticoagulation therapy practices. Kelly Rudd, PharmD, BCPS, CACP, NYS Chapter President of the American College of Clinical Pharmacy and Clinical Pharmacy Specialist, Anticoagulation at Bassett Medical Center, will provide an overview of new oral anticoagulants, potential complications, and related safety precautions. Additionally, Victoria Agramonte, RN, MSN, Project Manager, IPRO, will discuss the integration of practices that may reduce venous-thromboembolism and potentially preventable readmissions. Practical tools and resources will be provided to support hospitals anticoagulation safety and VTE reduction efforts.
