NYSPFP Readmissions Webconference: Pilot Phase 3 and Medication Reconciliation at Discharge

This session is the sixth in a series of eight educational web conferences focused on the 2014 Readmission Reduction Pilot Project. Jeffrey Schnipper, MD, MPH, Director of Clinical Research at Brigham and Women’s Hospital and Co-Chair of Partners Clinical Performance: Transitions in Care, will highlight best practices and strategies for conducting medication reconciliation at discharge. Dr. Schnipper will also discuss the important role of the hospitalist in the discharge process. In addition, NYSPFP staff will provide hospitals with tips for evaluating their Phase II findings and lessons learned, and will also present a sample team action plan for Phase III of the pilot. NYSPFP encourages all hospitals, even those not participating in the readmission reduction pilot project, to join the web conference.
