NYSPFP Readmissions Webconference: Patient & Caregiver Education and Empowerment

This session is the fifth in a series of eight educational web conferences focused on the 2014 Readmission Reduction Pilot Project. Pam Dardess, Principle Researcher for the Health Program at the American Institutes for Research, will provide an overview of the AHRQ Guide to Patient and Family Engagement Strategy 4 which focuses on Care Transitions from Hospital to Home and IDEAL discharge planning. Pam will discuss the basic principles and strategies to help hospitals develop effective relationships with patients, family members and caregivers and engage them in the discharge planning process to improve care transitions and reduce readmissions. As part of this process, the April 8 session will provide hospitals with a better understanding of how to validate the patient’s understanding of their care plan using the teach-back technique as one communication strategy.

NYSPFP encourages all hospitals, even those not participating in the readmission reduction pilot project, to join the web conference.

(begin logging on 15 minutes prior to start time)

• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.
