NYSPFP Readmissions Webconference: Engaging a Multidisciplinary Clinical Team

This session is the fourth in a series of eight educational web conferences focused on the 2014 Readmission Reduction Pilot Project. NYSPFP staff will discuss strategies for evaluating hospital’s Phase 1 pilot activities and provide a detailed overview of Phase 2 activities which includes key interventions for readmission risk mitigation during the hospital stay. The session will also include a hospital presentation on specific approaches to engaging a multidisciplinary clinical team in their readmission reduction efforts to ensure that roles, responsibility and accountability are established for every patient on the pilot unit who are identified as being at risk for readmission. NYSPFP encourages all hospitals, even those not participating in the readmission reduction pilot project, to join the web conference.



How to join the Webconference

(begin logging on 15 minutes prior to start time)

2. Enter your name and e-mail 
3. Enter the case-sensitive password: Prevent325
4. Click the "Join Now" button.
5. Fill in any additional information requested.
6. Click the "Submit" button at the bottom right-hand side of your screen.

After loading, a pop-up box will appear with the following dial-in instructions:

• The toll-free phone number: (877) 668-4493 
• The Meeting ID number: 646 790 625
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.
