NYSPFP Readmissions Webconference: Pilot Phase 1 Begins and Medication Reconciliation on Admission

As hospitals begin phase 1 of the NYSPFP Readmissions Pilot Project, this program will focus on key activities and the rapid-cycle improvement approach to change. NYSPFP is also pleased to welcome Anne Myrka, BS Pharm, MAT, BCPS, Pharmacist, Healthcare Quality Improvement Program, IPRO, who will review medication reconciliation issues on admission, and team accountability, collaboration and communication related to medication risk mitigation activities. Representatives from Crouse Hospital will discuss pharmacy involvement in medication reconciliation on admission for prevention of adverse drug events, and preventable readmission.

• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.
