NYSPFP Readmissions Webconference: Mitigating Risk Factors for Discharge Planning on Admission

This webconference will focus on the hospital admission risk assessment and the corresponding initiation of discharge planning strategies to mitigate the risk for readmission. Representatives from St. Peter’s Hospital and John T. Mather Memorial Hospital will discuss how they have identified a high-acuity pilot unit to assess patients at risk for readmission and implemented specific interventions during the hospitalization to mitigate these risks as much as possible. In addition, NYSPFP staff will review the goal and key processes for mitigating risk factors when planning the discharge process on admission.

5. Fill in any additional information requested.
6. Click the “Submit” button at the bottom right-hand side of your screen.

• The Meeting ID number: 649 017 721 
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.
