Readmissions Webinar # 1 - Identify High-Risk Patients and Appropriate Interventions

This Web conference will focus on the first phase of the Readmissions Pilot Project and identify specific interventions that can be implemented during the hospital admission process to help prevent readmissions. NYSPFP is pleased to feature Mark V. Williams, MD, Director, Center for Health Services Research, Professor and Vice Chair Department of Internal Medicine, and Interim Chief, Division of Hospital Medicine at the University of Kentucky, who will discuss evidence-based high-risk factors for readmission. Identifying patients at risk for readmission is a key intervention during the hospital admission process to provide the care team with the information necessary to initiate a comprehensive patient-centered care plan.

During the Web conference participants will receive detailed information about the requirements of Phase 1, as well as the timeline for participation. All NYSPFP hospitals, including those that have previously opted out of the readmissions initiative, are encouraged to participate.
