NYSPFP 2014 Preventable Readmissions Kickoff Webconference: A Rapid-Cycle Pilot Project to Reduce Avoidable Readmissions

This Webconference marks the beginning of the 2014 NYSPFP approach to addressing preventable readmissions. During the event, NYSPFP staff will introduce a new rapid-cycle improvement pilot project focused on opportunities to improve patient care by reducing preventable readmissions. Participants will receive detailed information about the requirements of the project, as well as the timeline for participation. All NYSPFP hospitals, including those that have previously opted out of the readmissions initiative, are encouraged to participate.

How to join the Webconference

(begin logging on 15 minutes prior to start time)

4. Click the "Join Now" button.
5. Fill in any additional information requested.
6. Click the "Submit" button at the bottom right-hand side of your screen.

After loading, a pop-up box will appear with the following dial-in instructions:

• The toll-free phone number: (877) 668-4493
• The Meeting ID number: 642 820 882
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference)

You will need to key this number into the telephone to ensure your computer and telephone are linked
