Preventable Readmissions Educational Webconference: Community-based Care Transitions Program

The complexity of reducing preventable readmissions requires communication and collaboration across all care settings. The Community-based Care Transitions Program (CCTP), created as part of the Affordable Care Act, is testing models of care to strengthen transitions from the hospital to other care settings for high-risk Medicare beneficiaries. There are currently twelve CCTP organizations in New York State. This Webconference will provide insight into the various state models, and their impact on reducing preventable hospital readmissions. Featured speakers will be the Eddy Visiting Nurse Association CCTP, Albany Memorial Hospital, and the Brooklyn Care Transitions Coalition.

6. Click the "Submit" button at the bottom right-hand side of your screen.

(Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.)
