Reducing Preventable Readmissions: The Hospital to Nursing Home Transition Web Conference

This Webconference will highlight a nationally implemented quality improvement program, INTERACT (Interventions to Reduce Preventable Acute Care Transfers) that offers opportunities to strengthen hospital partnerships with long term care providers to reduce preventable hospitalizations and readmissions. Laurie Herndon, Senior Project Coordinator for INTERACT, will present the INTERACT program and provide an overview of clinical and operational strategies and tools to aid in the early identification, assessment, documentation, and communication of significant changes in a patient’s status at different stages of care. Ms. Herndon will also discuss advance care planning tools and palliative care resources available to engage interdisciplinary teams in practice and system-based innovations that will improve patients’ quality of life and help reduce preventable hospitalizations.

   Your unique Attendee ID number: ____ (Assigned when you log into the Webconference)
