Preventable Readmissions Educational Webconference: New York State Behavioral Health Readmissions Quality Collaborative

This Webconference will provide an overview of the New York State Hospital Quality Collaborative on Reducing Behavioral Health Inpatient Readmissions focused on reducing readmission rates among adults discharged from behavioral health inpatient services. For many hospitals, behavioral health admissions account for a large portion of their readmissions. Hospitals are encouraged to consider participating in this important statewide initiative, if they do not already do so. This Collaborative is co-sponsored by the New York State Office of Mental Health (OMH), Greater New York Hospital Association (GNYHA), and Healthcare Association of New York State (HANYS).

How to join the Webconference

(begin logging on 15 minutes prior to start time)

1. Click on the following link to begin logging on: https://nyspfp.webex.com/nyspfp/onstage/g.php?d=645188852&t=a
2. Enter your name and e-mail
3. Enter the case-sensitive password: Quality123
4. Click the "Join Now" button.
5. Fill in any additional information requested.
6. Click the "Submit" button at the bottom right-hand side of your screen.

After loading, a pop-up box will appear with the following dial-in instructions:

• The toll-free phone number: (877) 668-4493
• The Meeting ID number: 645 188 852
Your unique Attendee ID number: ____ (Assigned when you log into the Webconference)

You will need to key this number into the telephone to ensure your computer and telephone are linked

This Webconference will be presented with Closed Captioning.
