Integrating Patient Safety Practices into the Care Delivery System

This webinar will highlight strategies for integrating patient safety practices associated with the prevention of hospital-acquired pressure ulcers, patient falls, and other adverse events into an infrastructure for patient-centered care delivery. These practices emphasize multidisciplinary teamwork and patient and family engagement, including multidisciplinary purposeful rounding, change of shift reports at the bedside with patients, and the use of daily goals setting worksheets. Hospitals’ efforts in embedding these practices within their systems to effect continuous improvement and patient-centered care will be featured.

• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.

NYSPFP encourages all NYSPFP hospital liaisons, and nursing-centered initiative team and data leaders to participate. Quality improvement professionals, nursing executives, nurse managers, physicians and other medical staff leadership, front line staff, case managers, nutritionists, patient advocates, and ancillary services are also encouraged to participate.
