Preventable Readmissions Educational Webconference: Transitions and Handoffs

This Webconference will focus on improvement strategies for patient transitions and handoffs. Amy Boutwell, M.D., M.P.P., President of Collaborative Healthcare Strategies, will highlight the key components necessary for an effective transition from the acute care setting, including: comprehensive medication reconciliation, adequate discharge instructions, follow-up phone calls, and post-discharge appointments with primary care providers. Dr. Boutwell will also discuss the evidence behind why these strategies are important, and how they are linked to patient outcomes.

How to join the Webconference

Begin logging on 15 minutes prior to start time:

5. Fill in any additional information requested.
6. Click the "Submit" button at the bottom right-hand side of your screen.

• The Meeting ID number: 646 615 244
• Your unique Attendee ID number: ____ (Assigned when you log into the Webconference) You will need to key this number into the telephone to ensure your computer and telephone are linked.
