The New York State Partnership for Patients (NYSPFP) is a joint initiative of the Healthcare Association of New York State (HANYS) and the Greater New York Hospital Association (GNYHA). NYSPFP was created in 2011 to participate in the Centers for Medicare & Medicaid Services’ (CMS) Partnership for Patients initiative to improve the quality and safety of health care provided in hospitals across the State.
In September 2016, CMS awarded a Hospital Improvement Innovation Network (HIIN) contract to NYSPFP. The HIIN initiative is a continuation of CMS’ Hospital Engagement Network initiative. NYSPFP is now partnering with IPRO, the State’s Quality Improvement Organization (QIO), to continue supporting participating hospitals’ improvement work with education, tools, resources, and direct project management support. Under the HIIN initiative, NYSPFP will build on participating hospitals’ achievements to meet the CMS goals of reducing hospital-acquired conditions by 20% and readmissions by 12% from a 2014 baseline.

This Web site provides hospitals with valuable resources from the NYSPFP initiative, including initiative overviews, meeting materials, and tools and information to assist providers with their efforts to sustain improvements made throughout the project.
